If you’ve never experienced chiropractic care before, you may not know what to expect when you come to our office for the first time. This section of the website will help you learn the process of becoming a new patient in our office. In this area of the website, you will be able to take care of the items necessary for your chiropractic visit prior to arriving, in the comfort of your own home. We have a variety of services in our office that allow us to individualize care for pain relief, health and wellness, sports performance, health concerns, and more. We look forward to sharing those options with you.

Online Forms

ChiroHealth Rockford offers our patient forms online so they can be completed in the convenience of your own home or office, before you even arrive to our chiropractic office. We ask that you return these forms 24 hours prior to your new patient appointment. This lets us know the history and current state of your health. What questions, concerns, and goals regarding wellness can we help you with? Let us know!
